Do I need to fill in the blank centre recognition activity report?

You don't need to fill out the blank centre recognition activity report. The report is designed to give you an understanding of what the activity entails so you can prepare for the visit.  

It’s sent to you three weeks before the visit and the final report is created by the assigned EQA and sent to you eight days after the completed visit.
